Change Form Layout?

I'm trying to design a form for web that will collect user data. Is it possible to change the form layout to have more than one field on a row?

For example, "last name", "first name", "middle initial" could be on one row, then "street address" would be on another row, and "city", "state", "zip" would be on another row.
