Click here or select the Customer Tracker template when creating a new workbook.

The New Customers screen below allows app users to add new customers via the [+] Add button in the upper right corner that you can see in the screenshot above.

The form below can be filled out and submitted. Once submitted, the user will be taken back to the previous screen (the Customers screen) where they will be able to see the new customer in the list.
