Survey Using Header Rows

Hi all, I'm trying to figure out if this is possible. I'd like to make a survey using header rows instead of columns, so that my results can be more easily printed on standard letter paper. Does anyone know if that's possible?

I've faced this issue before due to specific business logic. There are times when each survey response needs to store as it's own record/row instead of at a column level. The short answer is yes it's possible however after reading your post it seems your reasoning is for printing purposes only.

With that in mind you can accomplish printing with the standard column approach which would be the easiest solution. Here's an example of a survey table that stores the account name and survey responses in my example the survey has 5 questions, you'll notice the form displays the fields vertically which I believe addresses your printing issues.
